About this role
As a Carer Support Adviser your responsibilities would include:
- Managing conversations with carers to identify need, with assessments and support plans
- Providing information, advice, and wellbeing support
- Running support groups and activities
- Working with community groups and partnership working to identify Carers in Medway
About you
To be successful in this role you will need
- Experience of working with adults either in a 1:1 or group setting
- A working knowledge of health and social care particularly in relation to adults
- Excellent communication and interpersonal skills
- A high standard of written skills is needed in order to help the carer identify their support needs and to complete statutory assessments building resilience
